Responsible for the delivery of patient care through the nursing process of assessment, diagnosis, planning, implementation and evaluation. Coordinates and provides age-specific care for persons with wounds including but not limited to pressure ulcers, vascular ulcers, diabetic ulcers, trauma, STRN, skin tears, etc. Educates staff, families, patients, and the community about specialized care relating to wounds and dressings. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and achievement of desired patient outcomes. Collects data for care improvement opportunities.
