The RN’s purpose is to provide quality nursing care to patients, coordinate care with ancillary staff, and communicate effectively with physicians. Responsible for patient outcomes meeting high standards of care at a safe practice level set by TGH.
Roles Responsibility
Act as a clinical and/or emotional resource for clinical team, provide guidance for complex patients, perform thorough physical & psycho-social assessments, identify potential barriers for social determination of health, manage patient medication profile, communicate with provider lab & test results, complete triage/in-basket messages, annual wellness visits, care management & coordination of chronic conditions, provide patient education, maintain records of medical and nursing treatments, identify gaps in care & contribute to improving team's quality metrics, maintain patient experience and safety following emergency clinical policies and protocols.
