The Nurse Practitioner works independently with collaborative supervision using intuitive skills when problem solving in complex clinical situations.He/She demonstrates a highly skilled analytical ability in novel circumstances. He/She is credentialed to function in an expanded nursing role. He/sheis responsible for assessment and management of patients, including diagnostic and therapeutic interventions and the implementation and evaluationof an appropriate plan of care. All activities are performed within the guidelines of protocols agreed upon by the Medical Director and under medical supervision. He/She participates in clinical research and/or the utilization of its results. He/She is involved in multidisciplinary consultation for the delivery of patient care. All nursing practice is based on the legal scope of practice, ***** and specialty nursing standards, CNH Policies and Procedures, and in accordance with all applicable laws and regulations. The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. CNH supports the development of RN relationships within the community; specifically, those relationships associated with the health and well being of the community at large.

(Required)Basic Life Support for Healthcare Provider (BLS) (Required)Certified Pediatric Nurse Practitioner (CPNP) (Required) OrFamily Nurse Practitioner-Board Certified (FNP-BC) (Required)Functional AccountabilitiesClinical/Practice Assess individual patient clinical status and well-being by obtaining complete medical history and examination; developmental assessment; ordering appropriate laboratory tests, and x-rays as indicated. Gather and maintain patient medical records including lab, MRI, EEG results, and abnormal lab findings; interpret results and plan interventions accordingly. Collaborate with other health care team members, clinics and community agencies; address patients care needs. Develop and implement individualized plans of care, based on existing protocols and individual patient and family needs, including continuity of care issues. Perform minor procedures when indicated (e.g. ear irrigation, suture removal). Independently prescribe legend drugs according to guidelines. Provide clinical expertise for patients, families, and team members. Serve as a role model in clinical practice for complex patient care issues. Direct management of patient in emergency situations Enhance professional nursing by participating in professional organizations and committees. Participates in on call rotation for individual practice as needed Communication/ Consultation Act as a consultant in areas in clinical expertise to other health care professionals and to the community. Utilize both written and verbal means to thoroughly document and communicate patient status. Manage conflict constructively. Communicate needed system changes effectively . Identify needs for professional growth of staff and communicate these effectively . Management/ Administration Actively participate in the development of standards of care. Provide input and feedback to others for individual patients and their families to evaluate the quality of care. Organize workload in order to set priorities to facilitate short and long term goals. Consult with other health care providers regarding policies of patient care. Interpret the intent of the policies of the company in an understandable and non-biased manner to staff and families. Quality Assurance Monitor and document effectiveness of patient care interventions. Participate in QA monitoring with the department or Practice. Propose and participate in the development of patient care programs to improve the delivery of care. Patient and Family Assess learning needs of the patient and family. Implement teaching strategies using appropriate tools/resources/research. Evaluate patient and family level of understanding following implementation of teaching. Communicate patient and family learning needs and response to teaching to other members of the health care team. Develop educational materials directed at patients and families in area of clinical expertise. Research Disseminate information pertaining to the research of other professionals in order of specialization. Professional Education Participate in development of NP role and practice within organization. Participate in NP certification maintenance program. Attend continuing education conferences/lectures. Hold membership in specialty organizations. Develop and present educational programs to hospital and community health care professionals by Identify learning needs of staff. Develop appropriate in-services/projects to address identified needs. Meet periodically with clinical educators, services directors, departmental chairs, to assess staff needs.
