*****Park Medical Center is seeking an experienced RN to join the Shipley Cardiothoracic Center in the role of Nurse Navigator. Facilitates a timely, integrated and coordinated approach to the care of patients with surgical cardiac and thoracic disease. Coordinates care for patients throughout the continuum of care from diagnosis to the release from care. Acts as a liaison between the patient, staff, and referring physician offices. Develops and evaluates educational activities, performs in various roles like clinician, facilitator, advocate, consultant, and researcher.
