Nurse Clinical Program Coordinator – Full Time – Division of Ambulatory (various clinics)


The Ambulatory care clinics, centers and services are physician managed/directed services. However, the Executive Director for Ambulatory Services along with the Executive Director for Ambulatory Nursing and Clinical/Practice Managers collaborate with the physician staff to identify and define those areas where nursing care or nursing support is provided. Nursing care/Medical Support and services are provided in a highly collaborative, multidisciplinary and often multi-service environment. Generally children who experience acute onset of illnesses will be seen at Children’s ***** for initial treatment. Subsequent visits may occur in either the clinics or Regional Outpatient Centers. The type of service or care provided in each setting dictates whether nursing care or nursing services are provided. Each clinic or ROC has identified the diagnoses which will be seen at Children’s *****. Ambulatory Care Services consist of the specialty clinics and the primary care clinics, serving patients aged from infants to adults.

The Clinical Program Coordinator will successfully complete the hospital and nursing orientation at CNMC. Will coordinate nursing care and services for patients and family members in specific healthcare specialty encounters by telephonic, electronic, in person, and/or by mail. Will consult with physicians, and other healthcare providers to resolve health concerns.All nursing practice is based on the legal scope of practice, ***** and specialty nursing standards, CNMC Policies and Procedures, and in accordance with all applicable laws and regulations. The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. CNMC support the development of RN relationships within the community, specifically those relationships associated with the health and well being of the community at large. Responsible to lead the activities of the patient care delivery team for specific patient population. Provide continuous accountability to assess, plan, implement and evaluate clinical care and ensure that continuity of care is maintained.
