The HSC Pediatric Center (HSCPC) Case Manager oversees patients’ progression through the continuum of care while at HSCPC through individualized care coordination from referral/admission through discharge. The Case Manager actively coordinates the needs of patients who meet medical admission criteria. Responsibilities include patient assessments, identification of discharge goals, case management planning, and implementation of interventions, monitoring/tracking care progression, and ensuring follow-up and coordination of services. The Case Manager will partner with referring providers, and HSCPC staff and collaborate with payers and community vendors/resources to develop and facilitate a safe, effective, and efficient discharge plan of care. All nursing practice is based on the legal scope of practice, ***** and specialty nursing standards, HSC Policies and Procedures, and applicable laws and regulations.

Referral Management, Acts as liaison, Develops and maintains relationship, Identifies candidates, Performs thorough assessment, Develops comprehensive care plan, Facilitates tours, Serves as contact person, Markets and networks, Collaborates with designee, Ensures accurate insurance information, Collaborates with medical home and care management team, Coordinates and collaborates with physicians, Identifies barriers, Supports family self-management, Collaborates with patient's care team, Accepts and facilitates care transition hand-offs, Works with payers and care providers, Coordinates obtaining discharge medications and equipment, Supports coordination of discharge needs, Escalates barriers/challenges, Develops and maintains relationships with clinical staff and community resources, Identifies, implements, and evaluates processes to improve customer service, reimbursement, and hospital performance, Participate in identification of PI issues, Promote the development of the CRM department and healthcare team, Demonstrates understanding of service quality, Collaborates with co-workers, Innovates through improvement of care and efficiency, Dedicated to performance excellence, Embraces changes/improvements, Actively participates in new/improved programs and technology, Identifies and selects alternative solutions, Maintains a high level of activity/productivity, Anticipates problems, May expose to blood-borne pathogens occasionally, Full access to protected health information, Requires occasional travel.
