Advanced Practice Provider (APRN/PA)- Primary Care PRN

The advanced practice provider (APP) will provide primary care services, covering Primary Care offices and the mobile unit. Responsibilities include in-person and virtual patient visits, working autonomously and as part of an interdisciplinary team, ensuring quality metrics, HCC coding, patient satisfaction, and chart closure data. After hours and weekend calls are required.
