An exempt position responsible for providing care to patients enrolled within TGH ***** at Home, which is a unique and innovative department at TGH that provides inpatient ***** level care at the patient’s home with management of acute illnesses that may or may not be further complicated and/or exacerbated by complex comorbidities.
Roles Responsibility
Conducts history/physical examinations, admission workups and medicine reconciliation including assessments and plans in collaboration with supervising physician, Responds to house calls and evaluates patients as needed for pre/post-procedure complications, change in medical condition or other patient/RN concerns, Daily virtual and/or in person rounding on ***** at home patients, reviewing patient data, monitoring therapy and providing continuity of care, Counsels patients/family on condition and pre/post-surgical care, Documents patient information and care in the medical record including H&Ps, progress notes, and discharge summaries, Facilitates discharge planning and patient throughput from admission.
