The Nurse Navigation Coordinator at Montgomery Cancer Center coordinates referrals, patient care, testing and follows up with physicians, nursing staff and other ***** professionals, including internal and external ***** care providers to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources. Coordinator will also assess at risk populations to identify individuals who may benefit from ancillary service providers. The coordinator has oversight for the clinical nurse navigators and registered dietitians.
