To monitor and improve the case management process. To liaise with internal and external stakeholders, participate in complex case reviews, manage discharge and post-discharge activities and ensure accurate documentation and compliance with regulations. To foster a collaborative team environment, lead performance initiatives, assist with budget management and formulate departmental policies.
