***** ***** has an opening for a Program Manager for the Collaborative Care Model team. This is a grant funded position through a five-year grant from the Substance Abuse and Mental Health Services Administration (SAMHSA), in collaboration with the Minnesota Department of Human Services.

SUMMARY

The Collaborative Care Program Manager is responsible for developing and managing the Collaborative Care Model for integrated behavioral health primary care. This role will work closely with program leadership, operational leadership, and primary care teams at ***** *****. The Program Manager will lead day to day administration of the Collaborative Care Model grant, including scheduling meetings, connecting with key stakeholders, implementation of the work plan, and budget management in collaboration with Project Lead (Dr. Tyler Winkelman). The Program Manager will supervise at least five behavioral health clinicians (licensed mental health professionals) in working directly with patients and primary care teams. The role is housed within the Primary Care Care Ring and will report to the Senior Director for Primary Care.

RESPONSIBILITIES – Program Manager

In collaboration with the implementation and clinical team:

Establishes goals, objectives and standards for specified ***** program and/or services. Identify trends, new ideas or modifications needed to enhance program
Develop, plan, manage and coordinate delivery of services to patients
Develop and implement procedures, protocols and policies for program
Develop and analyze program initiatives for their effectiveness. Implement quality improvement activities
Develop, direct and maintain reporting guidelines and support systems to assist with program initiatives
Manage budget for program
Select, orient, supervise and evaluate performance of direct reports

Desired Qualification
Bachelor of Science in Nursing,Three or more years of clinical experience,Two or more years of program planning experience,Master's degree in related field preferred,LMFT, LICSW, or LPCC license preferred
