PRN RN Case Manager – Home *****

Services will be provided in accordance with current clinical practice guidelines and accepted professional standards of practice. Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement. Duties include covering/rotating weekends, after hours visits, and traveling based on patient caseload needs.
